How do I Apply?

Please follow the instructions below.

Create a short video addressing one of the following two questions below (your choice):

1. Explain the importance of your major in today’s society.

2. Discuss an accomplishment or event that sparked a period of personal growth for you.

a. Your video should be from 4 to 6 minutes in length.

b. Post your video to YouTube. Do not tag it or put Meridian Trust North Star Foundation or Scholarship in the name.

c. Fill out the application form below. 

If applicant is under 18, please submit a Parental Consent Form. Once signed please upload the Parental Consent Form to either the "Upload Document" or  the"Upload Image" button below.

OR

Please be sure to include your YouTube link to your video before clicking the submit button below once you have completed the application.
